How to Prepare for Your DOT Physical: A Driver’s Checklist

A DOT physical determines whether you’re medically qualified to keep driving, and keep working. Walking in unprepared is one of the most common reasons drivers end up with a shorter certification period, a required follow-up, or in some cases, a disqualification that could have been avoided with a little preparation.

Here’s what to do before your appointment so the exam goes smoothly and you walk out with your medical card in hand.

Before You Schedule

Know your expiration date. Your current DOT medical card shows an expiration date. Schedule your renewal exam a few weeks ahead of that date rather than waiting until the last few days. If a medical examiner flags something that needs documentation from your regular doctor, you’ll want time to get it before your card lapses.

What to Bring to Your Appointment

  • Your valid driver’s license or photo ID
  • A current list of all medications and dosages, including over-the-counter supplements

  • Any documentation related to ongoing medical conditions

Managing Common Conditions Before Your Exam

Certain conditions come up often during DOT physicals and can affect your certification if they aren’t well-documented or managed.

High blood pressure. If your blood pressure is elevated on exam day, even from appointment stress, you may be certified for a shorter period or asked to bring updated readings. If you have a history of high blood pressure, bring recent readings from your primary care provider or pharmacy blood pressure checks.

Diabetes. Drivers with diabetes, especially insulin-dependent diabetes, may need additional documentation from their treating physician. Bring your most recent A1C results and a letter from your doctor if you have one.

Sleep apnea. If you use a CPAP machine, bring documentation of compliance, such as a usage report, since examiners may ask about this directly.

Vision and hearing. If you wear corrective lenses or hearing aids, bring them and wear them to the exam. Trying to pass the vision or hearing screening without them can result in a failed screening that’s easily avoidable.

The Night Before

Get a normal night’s sleep and avoid excessive caffeine or energy drinks right before your appointment, both can temporarily elevate blood pressure and heart rate. Stay hydrated, and eat a normal meal beforehand. Skipping meals before a physical can sometimes affect vital sign readings.

What Happens If You Don’t Pass on the First Try

Not passing on the first attempt doesn’t necessarily mean you’re disqualified from driving. In many cases, it means the examiner needs additional information, a follow-up on blood pressure, updated labs, or documentation from a specialist, before issuing a full certification. Understanding what’s being asked for and getting it promptly is usually the fastest path to getting your medical card issued.
